When she hold the Commissioner's office door open for him. Which may mean a canon name change or since the actress' name is Abigail, a mistake that was not caught in editing. However in 3.19 - Loss of Faith, Frank Reagan says "Goodnight Abigail."  In no episode has Detective been called Melissa, it may have been the wrier's intention use the name Melissa in Season one but Canon is now Detective Abigail Baker

Abigail Baker is a NYPD detective who currently serves the primary aide to the commissioner. As such, she is also a member of the Police Commissioner’s Squad. Baker is responsible for preparing and accessing Frank Reagan's official schedule. As such, Baker acts as the final barrier between anyone who wishes to speak or meet with the commissioner as nobody gets to do either without her approval. Baker also assists Frank in the transmission of his communications, that comes directly from his office.
